چکیده

Pneumobilia, or air within the biliary tree, is a poor prognostic indicator in a patient without prior biliary sphincterotomy. Differential diagnosis includes infection with gas-forming organisms, choledochoenteric fistula in the setting of gallstones or penetrating ulcer disease, malignant invasion from a primary liver or biliary tract tumor, or metastatic disease. Treatment depends on etiology and patient factors, but often requires surgical intervention. We report a patient with gastrointestinal bleeding in whom pneumobilia was incidentally noted on abdominal plain film. Computed tomography and endoscopy revealed the biliary-enteric fistula to be caused by metastatic colon adenocarcinoma invading the biliary tree.

A case of choledochoduodenal fistula – an unusual case report

Choledochoduodenal fistula (CDF) is an abnormal communication between the choledochus and the duodenum, accounts for 5-25% of all internal biliary fistulas. Here, we report a case of CDF secondary to chronic duodenal ulcer who presented with cholangitis. CDF is suspected in case of pneumobilia, and surgery is recommended for refractory cases.

Choledochoduodenal fistula after biliary placement of a self-expanding metallic stent for palliation of pancreatic cancer.

A 56-year-old manwith pancreatic cancer underwent palliative placement of a biliary self-expanding metallic stent (SEMS) for obstructive jaundice after chemoradiation (nine cycles over 5 months). He presentedwithmelena but denied abdominal pain, nausea/vomiting, or hematemesis. Nodules suspected of being metastases were noted in both lung fields. The p...

Spontaneous Choledochoduodenal Fistula after Metallic Biliary Stent Placement in a Patient with Ampulla of Vater Carcinoma

Biliary stent-related enteric perforations are very rare complications that are caused by the sharp end of a metallic stent, stent migration, or tumor invasion. Moreover, the choledochoduodenal fistula resulting from metallic biliary stent-induced perforation is extremely rare. Here, we report a case in which a spontaneous choledochoduodenal fistula occurred after biliary metallic stent placeme...

Choledochoduodenal fistula in the setting of Crohn’s disease

Of all the spontaneous fistulas that occur between the extrahepatic biliary system and the intestine, a choledochoduodenal fistula is rarely seen. When it does occur, it is most often secondary to a perforated duodenal ulcer, choledocholithiasis, or cholelithiasis. It may also be seen following complications related to iatrogenic injury or tuberculosis. Generally, choledochoduodenal fistulas ar...
